Our Location & Facilities

Peak Form Physio + Wellness has two locations serving the Mississauga community.

Our Port Credit location is at 515 Lakeshore Road East near Cawthra Road and the village of Port Credit. We are part of the District at Lakeview commercial complex, on the ground level beside BMO Bank. There is free covered parking on site.

We are located between the Port Credit and Long Branch Go Train stations, a six minute drive from either station. Peak Form Physiotherapy is also accessible by Mississauga Transit via the #23 Lakeshore bus.

Peak Form Physio + Wellness Clarkson is at 950 Southdown Road near Royal Windsor Drive. We are part of the Clarkson Crossing plaza, on the ground level beside the Life Labs. There is free parking on site.

We are located just south of the Clarkson Go Train station, a five minute drive from the station or a nice ten minute walk!

We're passionate about active lives and healthy living. Through physiotherapy, we want to make a difference, person by person, one community at a time. We want to do this by being role models for our clients and for the community of Port Credit and Mississauga. Above all, we strive to provide the best physiotherapy care to help people get back to doing what they love.

Our Team

David Sun, Registered Physiotherapist

David Sun

Registered Physiotherapist, Owner

Being involved at an early age in several sports including soccer, volleyball, and snowboarding, David has had his fair share of injuries, including a serious knee injury that required him to undergo extensive physiotherapy. This gave him a profound perspective on the rehabilitation process as a patient, which he has carried with him. This experience has allowed him to more clearly understand patients’ needs.

David is dedicated to providing quality patient-focused care through manual therapy, education, and patient specific therapeutic exercises. He believes that the key to successful treatment starts with a thorough assessment to determine the root cause of pain or injury. David believes in continuous learning and has training in Mulligan Techniques, McKenzie Treatment approach to low back and neck pain, Manual Therapy, acupuncture, and dry needling. David is also a member of the Toronto Written Development Group (WIGC) for the Canadian Alliance of Physiotherapy Regulators, where he is involved with the development of content for the Physiotherapy Competency Exam. In his free time, David enjoys cycling, travelling, and exploring the Toronto food scene.

Brenden Du

Registered Physiotherapist, Clinic Director, Co-Owner (Clarkson)

Brenden is the Clinic Director and Co-Owner of the Peak Form Physio + Wellness Clarkson Location. He is dedicated to helping people recover from pain and injuries, move better, and feel their best.

Brenden treats various conditions including sports injuries, post surgical rehab, and general aches and pains. He particularly enjoys working with neck and back pain, rehabilitating shoulder issues, and jaw pain (TMJ). He takes a hands-on, personalized approach to treatment, combining manual therapy and tailored exercises to determine the root of each problem and to support long-term results.

Brenden completed his Master's in Physical Therapy at the University of Toronto after obtaining a Bachelor's of Science in Kinesiology at McMaster University. As an avid learner, Brenden has gone on to complete advanced post-graduate training in orthopedic manual and manipulative therapy, including level 3 of the Orthopaedic Manual Therapy program and is currently working toward his FCAMPT designation. He's also trained to provide acupuncture, dry needling, and spinal manipulations.

Outside of the clinic, Brenden loves spending time outdoors with his dog. He enjoys hiking local trails and multi-day backcountry camping trips by foot or canoe.

Thomas Gospodarek

Registered Physiotherapist, Lead Clinician

Thomas had always been around sports from a young age and as a result had been a patient of physiotherapy himself. He understands how worrying and scary it can be to be injured. This gives him a deeper understanding of what patients are going through when they come to the clinic in pain.

Thomas went to Australia to complete a Masters of Physiotherapy from the University of Sydney in 2018. Since graduating Thomas has worked in private practice, which has given him the opportunity to work with a variety of populations and injuries. These include acute and chronic injuries, rehab pre and post surgery, repetitive strain disorders, and sports injuries.

Thomas’ approach to treatment is a combination of manual therapy, acupuncture and dry needling, and specialized exercise programs to manage and prevent injuries.

Thomas is also specialized in Concussion management having completed the Shift Concussion management program. This involves treatment for acute concussion symptoms as well as chronic post concussion symptoms such as vestibular issues, balance issues, headaches, nausea and visual tracking.

In his free time he’s likely in the gym, watching Formula one or basketball and planning the next renovation of his condo. Thomas is also Fluent in Polish.

Kenneth Ong

Registered Physiotherapist

Kenneth’s passion for learning about human movement and anatomy led to the completion of a BSc. in Honours Kinesiology at McMaster University, after which he completed his Master’s in Physical Therapy at the University of Toronto.

From a young age, Kenneth has always been heavily involved in competitive sports such as volleyball, rugby, and badminton. Naturally, in these years as an athlete, he has also had his fair share of injuries. Kenneth’s extensive experiences as a patient to the rehabilitation process has played a huge role in the development of his dedication to helping others return to the activities or sports that they love.

Treatment sessions with Kenneth will include a combination of manual therapy, movement and postural analysis, and exercise prescription. Most importantly, Kenneth aims to ensure that these treatment strategies are incorporated with a plan of care that has been tailored towards achieving each patient’s individualized goals.

Outside of work, you will often find Kenneth at the gym working on his personal strength and conditioning, at the volleyball courts playing pick up or at his local Muay Thai gym. Kenneth is also a huge basketball fan, being involved in multiple fantasy basketball leagues and watching every Raptors game whenever he gets the chance. Aside from sports, Kenneth also spends his down time relaxing or walking trails with his mini schnauzer Toby.

Alafyah Bharmal

Registered Physiotherapist, Pelvic Health Physiotherapist

Alafyah believes in delivering high-quality care with an evidence-based approach to help her patients succeed in their rehabilitation journey.

Alafyah earned her Master’s in Physical Therapy from the University of Brighton in England, where she gained a strong academic foundation alongside a global perspective on healthcare. Her diverse clinical experiences across Canada, UK, and Africa have fuelled her passion for empowering and supporting patients in achieving meaningful, long-term outcomes.

Alafyah’s treatment approach combines manual therapy with corrective exercises and holistic education to address a wide range of musculoskeletal injuries, chronic pain and activity related conditions. Her sessions are guided by the fundamental principles of patience, empathy and collaborative care. She is genuinely passionate about helping her patients regain confidence in their movement mechanics and take an active role in their recovery. She values lifelong learning and is currently aiming for acupuncture and pelvic health certification completion in the near future.

Outside of the clinic, Alafyah enjoys spending time outdoors, whether it’s hiking in nature, practising yoga or travelling to explore new cultures and cuisines. She is fluent in Hindi, Urdu, Gujarati and Arabic.

Aaron Koh

Registered Physiotherapist

Aaron completed both his Master of Science in Physical Therapy and Bachelor of Kinesiology at the University of Toronto. With a strong background in strength and conditioning, Aaron has had experience working with youth, collegiate, and professional athletes, as well as individuals of all activity levels.

Growing up playing provincial-level soccer, Aaron has a deep appreciation for the challenges of injury and rehabilitation. This experience continues to fuel his passion for helping others return to the activities they love.

Aaron takes an active, client-centred approach, creating tailored treatment plans that combine manual therapy, soft tissue techniques, targeted exercise, and education. His goal is not only to support recovery, but also to empower his clients to move with confidence and achieve their personal goals.

Outside of the clinic, Aaron enjoys staying active through competitive soccer and the gym, as well as exploring new restaurants, travelling, and spending time with friends and family.

Heao Sun

Registered Physiotherapist

Heao earned her Bachelor of Kinesiology from the University of British Columbia and her Master of Physical Therapy from Western University. During her studies, she completed an international clinical placement in Hawaii, where she gained valuable experience working with diverse patient populations and learning different treatment approaches practiced abroad.

Heao has extensive experience treating patients of all ages, from older adults to those recovering from workplace injuries. She is passionate about providing patient-centred care, developing individualized treatment plans that address each person’s specific goals and needs. Her approach combines manual therapy with targeted therapeutic exercises to support recovery, improve function, and promote long-term wellness.

Outside of the clinic, Heao enjoys travelling, playing tennis, and creating her own cosplays. She is fluent in Mandarin and is currently learning Japanese, as she enjoys learning new languages and exploring new cultures.

Dennis Tran

Registered Massage Therapist

From a young age Dennis was passionate about sports, you could always find him on the volleyball court or soccer field. This led him to pursue a BSc degree in Honours Kinesiology from the University of Waterloo to better understand human movement, anatomy, and health. After completing his degree, Dennis explored this passion further by attending the Sutherland- Chan School of Massage Therapy. Through clinical placements at various hospitals and rehabilitation institutions, he learned the importance of massage in stress, pain, and overall wellbeing.

Dennis takes a client- centered approach to treatment, prioritizing client needs and concerns. Every treatment plan will be specific to each person, ensuring all their goals and needs are met. His work also emphasizes self-care and patient education, to ensure healing continues beyond the treatment room.

Outside of the clinic, Dennis enjoys playing sports, running, and weightlifting at the gym. He is also always looking for the next concert to attend, and next restaurant to eat at.

Richard Chen

Osteopathic Manual Practitioner (M.OMSc)

Richard formerly worked as a scientist within the pharmaceutical industry while maintaining his hobbies in Olympic weightlifting and powerlifting. Through the injuries obtained from training and competition, and a desire to heal and optimize his body, Richard was able to experience a wide range of therapeutic modalities including osteopathy. In 2019 he enrolled at the Canadian Academy of Osteopathy with a focus on manual therapy and a comprehensive understanding of the body's structure and function. Richard completed the program in 2023, officially becoming an osteopathic manual practitioner.

Richard's personal experience as an athlete provided him with unique insights into the body's mechanics and the impact of physical performance. His dedication and commitment to learninga llowed him to develop a deep understanding of osteopathic principles and techniques.

Having completed his education, Richard is now equipped to make a meaningful impact in the field of osteopathy by looking at the body holistically. His goal is to help individuals address both the symptoms and root causes of their ailments, providing personalized care tailored to their specific needs. With a strong foundation in manual therapy and a passion for promoting wellness, Richard looks forward to applying his skills as an osteopathic manual practitioner to enhance the health and well-being of those under his care.

Kaitlyn Aguiar

Naturopathic Doctor

Kaitlyn is a licensed naturopathic doctor, dedicated to helping people feel vibrant, strong, and empowered in their health. Her approach blends evidence-based natural medicines with a deep respect for the body’s innate ability to heal. Kaitlyn’s areas of clinical focus include sustainable weight management, digestive health, stress support, and functional movement therapies. Combining naturopathic medicine with lifestyle interventions and functional training to support both the physical and emotional aspects of health.

With additional certifications such as facial acupuncture, lymphatic drainage massage, and a 6- year history of personal training, Kaitlyn offers a truly whole-body approach to medicine, supporting clients along their health journey.

Whether it’s uncovering the root cause of symptoms, creating personalized treatment plans, or guiding sustainable lifestyle shifts, Kaitlyn’s goal is to support and improve long-term health—naturopathically.

Shaffin Haji

Certified Pedorthist

Shaffin is a Certified Pedorthist with over two decades of experience in lower limb biomechanics, specializing in foot function, gait analysis, orthotic therapy, pediatric foot care, compression therapy and bracing. His clinical focus is centered on identifying the root cause of foot-related issues and improving overall movement through targeted support and correction.

Over the course of his career, Shaffin has assessed and treated thousands of patients, helping individuals of all ages address conditions ranging from flat feet and overpronation to chronic pain affecting the knees, hips, and lower back. His approach combines detailed biomechanical assessment with practical, results-driven solutions designed to improve comfort, alignment, and long-term mobility.

Shaffin has a strong interest in preventative care, particularly in early intervention for children, where proper support and guidance can influence long-term development and function. He is also the founder of NUKIX, an innovative platform focused on advancing children's foot health through technology and personalized care.

Outside of the clinic, Shaffin enjoys spending time with his family, staying active, and continuing to explore ways to improve patient care through innovation and education.

Leanne Chan

Owner, Clinic Operations

Leanne comes from a multi-disciplinary background of science, art, and business which stems from her curious nature and her genuine interest in lifelong learning. With years of experience educating and communicating various health and scientific concepts to both patients and healthcare professionals alike, she brings a wealth of knowledge and experience to the design, operations, and marketing at Peak Form Physiotherapy.

Leanne believes that the patient experience begins before you even enter the clinic. It starts from the moment you learn about us from your friend or doctor, or when you first stumble upon our website, or perhaps even the first time you meet us out in the community. To that end, she strives to ensure that all patients have a great experience at Peak Form Physiotherapy with consideration for every aspect from accessibility and facilities, to the team members involved in your physical rehabilitation, in order to make your every visit comforting, motivating, and a step in the right direction.
