The first visit will consist of a one-on-one assessment during which the physiotherapist will get a subjective history of your injury, followed by a thorough orthopaedic examination. This examination is essential in determining the root cause of your injury, so that we are treating the source of your pain, and not just the symptoms of it. After discussing their findings with you, the physiotherapist will create a personalized treatment plan specific to your injury to help you get out of pain and back to doing what you love.
The first appointment lasts approximately 1 hour, and will include treatment to help start you on your road to recovery. Please arrive 15 minutes prior to your scheduled appointment time, as you will be required to fill out our patient intake forms, also found online here.
How long are my follow-up appointments?
Follow-up appointments are booked on the half hour, consisting of either 30, 45, or 60 minutes of one-on-one time with the physiotherapist. Your treatment plan may also include a gym/exercise component, which will be in addition to the appointment duration. We never double book your appointments and we strive to stay on schedule. We respect your time, and we ask you to extend us the same courtesy.
What should I do if I have to cancel my appointment?
We understand that unforseen circumstances arise and you may need to cancel an appointment. We respectfully ask for appointments to be cancelled at least 24 hours in advance by email, phone, or through our online booking system. For same day cancellations or missed appointments, a standard missed appointment fee equal to the full fee of the scheduled appointment will be charged. Please note, this fee cannot be charged to your extended health care insurance plan.
Do I need a doctor's referral?
Physiotherapy is a primary care health profession which means you do not need a physician referral to see a physiotherapist. However, some extended health insurance plans may required a doctor's referral in order for physiotherapy services to be covered.
What should I wear to my appointment?
We recommend wearing loose-fitting, comfortable clothing - tank tops or t-shirts for the upper body, shorts or athletic pants for the lower body. Running shoes are recommended for any gym-based rehabilitation. Be prepared to move during your appointment with us!
Is there parking at the clinic?
Peak Form Physiotherapy is conveniently located at the Lakeview District Commercial Complex by Trinity. There is free covered parking on site.

VISA, Mastercard, debit, cash, and cheques are accepted.

Please note, individual plans may vary. Bring your insurance information with you and we can help determine if your plan is eligible for direct billing. If we are unable to bill directly, we will provide you with an official receipt at the end of each appointment and would be more than happy to help assist you with the claims process.
How do I get to the clinic?
We are located at 515 Lakeshore Road East near Cawthra Road in Mississauga, part of the new District at Lakeview commercial complex on the ground level beside BMO Bank. We are also located between the Port Credit and Long Branch Go Train stations, a six minute drive from either station and are also accessible by the Mississauga Transit #23 Lakeshore bus.
